Position Overview

The Personal Service Coordinator/Case Manager III plays a crucial role in assisting clients both in office settings and within the community. This position focuses on enabling clients to perform daily living tasks, thereby promoting their independence and integration into the community.

Key Responsibilities

  1. Collaborate with the Personal Service Coordinator Supervisor to implement the case management plan, including screening, evaluating, and executing specific activities outlined in the Individual Service and Support Plan (ISSP).
  2. Ensure accurate documentation of case records in compliance with Medi-Cal guidelines and contractual obligations.
  3. Participate in weekly supervisory meetings to discuss client progress and case management strategies.
  4. Deliver direct services to clients, which may include group counseling and individual support as specified in the ISSP and Plan of Care.
  5. Timely submission of all documentation and billing related to client services.
  6. Coordinate and facilitate services for clients with Severe Mental Illness (SMI).
  7. Evaluate potential clients to determine their needs for case management, rehabilitation, and residential services.
  8. Provide career planning and educational development services to clients.
  9. Assist clients in developing essential skills for maintaining employment and pursuing educational goals.
  10. Assess clients' readiness for work and develop tailored education or training plans.
  11. Offer classroom instruction or training to clients as needed.
  12. Create employment and work experience opportunities within the community.
  13. Establish and maintain supportive referral services for clients.
  14. Provide counseling to enhance clients' job-related skills.
  15. Maintain systematic communication with agencies and stakeholders involved in client rehabilitation.
  16. Report any unusual occurrences that may affect program operations or client safety to the Program Director.
  17. Complete a minimum of 40 hours of training annually as required by Turning Point.
  18. Respond effectively and efficiently in emergency situations.
  19. Drive for company-related business and transport clients as necessary.
  20. Physical presence at the office is mandatory.

Qualifications

  1. Bachelor's Degree in social work, psychology, vocational rehabilitation, criminal justice, or a related field from an accredited institution.
  2. At least one year of experience in managing rehabilitation for families facing significant barriers to independent community living is preferred.
  3. Familiarity with best practices for treating clients with Severe Mental Illness (SMI) and Dual-Diagnosis individuals.
  4. Proficient communication skills in both written and spoken English.
  5. Able to work effectively with diverse groups across various ethnic, cultural, and socioeconomic backgrounds.
  6. Certification in emergency first aid procedures is required.
  7. Intermediate proficiency in Microsoft Office Suite, including Word, Excel, Outlook, PowerPoint, and familiarity with communication platforms like Teams and Zoom.
  8. Possession of a valid driver's license and access to a reliable, insured vehicle.
  9. Ability to pass a criminal background check and pre-employment drug screening.
