Job Summary:

The Speech Therapist will direct, supervise, evaluate, and provide therapy services to patients in their homes as prescribed by the attending physician.


Responsibilities:
  • Conduct evaluations and complete reports in a timely manner
  • Plan and provide occupational therapy services
  • Record treatment and patient reactions on clinical/progress notes and incorporate into patient records weekly
  • Maintain accurate records on all patients, including summary reports
  • Utilize community resources and agency personnel for referrals
  • Attend case conferences as requested
  • Participate in staff development and in-service education
  • Confirm weekly visit scheduling with the Therapy Coordinator
  • Participate in discharge planning activities
