We are looking for a dedicated and experienced LPN to join our team of healthcare professionals. As a Home Health Nurse LPN, you will be responsible for providing personalized care to our patients, including:

  • Implementing physician-prescribed plans of care
  • Conducting patient assessments and coordinating care
  • Administering prescribed medications, treatments, and therapies
  • Documenting care provided in our system of record
  • Teaching and training family members on health promotion and disease management
Requirements

To be successful in this role, you must have:

  • An active, unencumbered LPN license in the state where you will practice
  • Current CPR certification from the American Red Cross or American Heart Association
  • A TB skin test (current within the last 12 months)
  • Six months of prior hands-on nursing experience (preferred but not required)
  • Reliable transportation
