The Cleveland Clinic's GYN Oncology department is a world-renowned leader in cancer care. We are seeking a highly skilled and compassionate Care Coordinator to join our team and help patients achieve optimal outcomes.

We are looking for a nurse with a minimum of 3-5 years of experience in oncology or a related field. A BSN degree and certification in oncology or a related field are also required. Strong communication and interpersonal skills are essential for this role.

Primary Responsibilities:

  • Develop and implement personalized care plans for high-risk patients.
  • Provide education and coaching to patients and families on disease management and self-care strategies.
  • Identify patients who require targeted outreach and coordinate interventions to improve outcomes.
  • Conduct comprehensive clinical assessments to identify patient needs and develop effective care plans.
  • Serve as a liaison and advocate for patients and families throughout the care continuum.
