About the Job
We are seeking a skilled Clinical Program Coordinator to join our team at University Hospitals. As a key member of our cardiovascular care team, you will play a vital role in ensuring the highest level of patient satisfaction and care coordination. Your primary responsibility will be to navigate patients through their entire healthcare journey, from pre-op to post-discharge. This includes collaborating with hospital staff, leadership, and committee members to achieve optimal patient outcomes and satisfaction.

Key Responsibilities:
  • Navigate patients through their healthcare journey from pre-op to post-discharge.
  • Collaborate with hospital staff, leadership, and committee members to achieve optimal patient outcomes and satisfaction.
  • Develop and monitor informational and educational material opportunities for patients and families.
  • Support operational initiatives and provide exceptional customer service as a resource to patients and families.


Requirements:
  • Bachelor's Degree in Nursing (BSN) or Associate's Degree in Nursing (ADN).
  • 5+ years of experience in clinical medicine, or 3+ years of experience in cardiology.
  • Ambulatory nursing experience preferred.
  • Current knowledge of cardiovascular disease management trends and care across different practice settings.
  • Understanding of hospital discharge processes.
  • Self-directed and motivated individual with strong communication skills.
  • Familiarity with Microsoft Office.


Salary Information: The estimated salary range for this position is $80,000 - $110,000 per year, depending on experience and qualifications.
