Job Summary:
The Clinical Pharmacist will play a vital role in optimizing patient medication therapy outcomes and quality of life by providing high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team.
Key Responsibilities:
- Develop and implement patient-centered care plans to optimize medication therapy outcomes
- Collaborate with the healthcare team to provide comprehensive patient care
- Provide education and support to patients and their families on medication use and management
- Conduct medication therapy reviews and provide recommendations for optimization
- Participate in quality improvement initiatives to enhance patient care and safety
Requirements:
- PharmD in Pharmacy or Bachelor's Degree in Pharmacy
- Oregon Pharmacist License
- Acute care hospital pharmacy experience or equivalent
- Completion of ASHP-accredited PGY1 residency (preferred)
