Position Overview

The primary objective of this role is to provide exceptional care to residents through a comprehensive assessment process, which includes evaluation, planning, intervention, and implementation. The duties will be overseen by the Woodlands Assisted Living Manager and guided by the Administrator, adhering to all relevant policies, procedures, and regulatory standards.

Key Responsibilities

This outline serves to highlight the essential functions and expectations of this position, while allowing for the assignment of additional job-related tasks as necessary.   1. Commitment to Care: Uphold the Cantata Promise by treating every individual—clients, colleagues, and community members—with respect, integrity, and compassion.
  2. Resident Safety: Monitor and assess resident well-being, ensuring effective communication of any changes in their condition to the clinical team and family members.
  3. Medication Management: Oversee medication ordering, preparation, administration, and monitoring in accordance with the established service plan.
  4. Service Plan Maintenance: Regularly review and update resident service plans to reflect observed changes and necessary adjustments.
  5. Staff Accountability: Ensure that Resident Assistants and Certified Nursing Assistants deliver care in alignment with the resident service plan.
  6. Education and Support: Provide guidance and information to residents and their families, promoting independence through wellness initiatives.
  7. Emergency Communication: Notify family members in cases of health emergencies or hospitalizations, ensuring the Woodlands Manager is informed.
  8. Documentation: Complete all required documentation accurately and promptly, including updates in the electronic medical record (EMR).
  9. Effective Communication: Maintain timely communication with the Woodlands Manager and other staff to ensure resident needs are met.
  10. Shift Coordination: Facilitate smooth transitions between shifts by ensuring proper communication and operational readiness.
  11. Resource Management: Ensure the availability of necessary supplies and equipment for resident care.
  12. Reporting: Prepare and deliver reports to healthcare providers to ensure coordinated care and follow-through on medical orders.
  13. Leadership: Act in the capacity of the Woodlands Assisted Living Manager during their absence.
  14. Adaptability: Demonstrate flexibility in responding to changing situations and needs.
  15. Professional Development: Participate in training and meetings as required by Cantata's policies.
  16. Policy Adherence: Execute tasks in compliance with departmental and organizational policies.
Interactions

This role involves extensive daily interaction with residents, as well as regular communication with personnel across all levels of the organization and occasional contact with family members and community representatives.

Authority and Accountability

As a Wellness Nurse, you are entrusted with the administrative authority, responsibility, and accountability necessary to fulfill your assigned duties.

Confidentiality and Accuracy

It is imperative to safeguard sensitive information, including resident health data and organizational records, while ensuring accuracy in documentation to avoid regulatory penalties.

Physical and Environmental Requirements
  1. Must meet health requirements as outlined by facility policies.
  2. Ability to lift, push, or pull a minimum of twenty-five (25) pounds.
  3. Availability for emergency callbacks and assistance in resident evacuations.
  4. Work may extend beyond standard hours, including weekends and holidays.
  5. Engage in various physical activities such as standing, walking, bending, and climbing as required.
Qualifications
  1. Valid certification as a Registered Nurse or Licensed Practical Nurse in the State of Illinois.
  2. Current CPR certification.
  3. Demonstrated leadership skills and ability to work collaboratively with others.
  4. Effective verbal and written communication skills.
  5. Capability to motivate and lead diverse groups.
  6. Ability to maintain positive relationships with residents, families, and staff.
  7. Proficient in making independent decisions when necessary.
  8. Attention to detail and basic computer literacy, including familiarity with EMR systems.
  9. Ability to manage the emotional and mental demands of the role.
  10. Experience working with diverse populations, including the elderly and those with disabilities.
