Key Responsibilities- Assist with direct patient care, including activities of daily living and other cares, under the guidance and delegation of the Registered Nurse.
- Obtain and record vital signs, including temperature, blood pressure, pulse, respirations, oximetry, and pain score, and notify the nurses as needed.
- Maintain knowledge and demonstrate competency in the use of patient equipment, including manual and automatic blood pressure machines, pulse oximeters, and other devices.
- Assist with patient treatments, such as repositioning, pre-operative hair removal, and shower preps, and empty all drains and collection bags.
- Record acquired data and observations resulting from patient care, including vitals, patient activity, tolerance, elimination, and nutrition, within the Electronic Medical Record (EMR).
- Identify and communicate significant changes in patient status to the Registered Nurse.
- Communicate and collaborate effectively with the Registered Nurse and other multidisciplinary team members, as well as patients, families, and significant others.
- Assist with patient dismissal process, including packing up belongings, dressing the patient, and escorting the patient to awaiting transportation.
- Assist the Registered Nurse with hourly patient rounds and respond to patient call lights in a timely manner.
- Initiate appropriate measures to ensure the safety and well-being of patients, families, visitors, and staff, and respond appropriately to Emergency Codes.
- Provide follow-up to meal service, collect trays, record fluid intake, calorie counts, and appetite assessment, and distribute HS snacks and document refusal.
- Maintain cleanliness, organization, and safety of the patient environment.
- Participate in intradepartmental meetings and in-services on the unit.
- Maintain competencies on an ongoing basis.
- Participate in improving patient safety and recognize and communicate issues/occurrences using the VOICE documentation system.
- Maintain confidentiality of information pertaining to patients, physicians, employees, and Trinity Health Corporation business.
- High school diploma or equivalent required.
- Indiana CNA license required.
- CPR certification on hire or able to attain within 60 days.
- 6 months experience as a Patient Care Assistant or Nurse Technician in an acute care setting or completion of 2 clinical semesters in nursing school.
- One year of experience in an acute/ambulatory environment preferred.
- Ability to respond with flexible and changing workload of patient assignments.
- Ability to perform as a team member, supporting a patient-focused environment in all interactions with patients, families, visitors, and volunteers.
- Knowledge of basic nursing practices.
- Knowledge of principles of body mechanics.
- Knowledge of methods of maintaining a safe, working environment.
