Job Title: Adult Health Home Care Manager

Liberty Resources is seeking a dedicated and compassionate Adult Health Home Care Manager to join our team. As a key member of our care management team, you will play a vital role in ensuring the health and well-being of adults in Cayuga County.

Job Summary:

The Adult Health Home Care Manager will work with adults who are eligible for and in need of care coordination to identify and address their physical, behavioral, and social health needs. You will establish an integrated care team of service providers and supports to reduce the risk of emergency room visits, hospitalizations, and other out-of-home placements.

Responsibilities:
  • Conduct ongoing assessments to identify strengths and needs, and develop comprehensive care plans that establish clear goals to improve health and wellness.
  • Refer individuals to healthcare providers, mental health and substance use providers, medications, housing, social services, and other community resources while coordinating with those providers.
  • Manage an average caseload of 30 clients and conduct home and community-based visits to engage members.
Requirements:
  • Minimum of a bachelor's degree with 2 years of relevant experience in health or human services; or master's degree with 1 year of relevant experience; or Registered Nurse with 2 years of relevant experience.
  • Previous care coordination/case management experience working with adults living with Serious Mental Illness, developmental disabilities, substance use disorders, or chronic medical conditions preferred.
  • Must have a valid New York State driver's license and access to reliable transportation.
