Role Description

This is a full-time hybrid role for a Corporate Licensed Social Worker in a dynamic and growing organization. The successful candidate will lead a team of Licensed Master of Social Workers (LMSWs) across the United States, focusing on policy and procedure creation, training, and onboarding new team members. In addition to the corporate role, the individual will maintain a caseload with a lower patient load compared to large dialysis providers.

Key Responsibilities

  • Provide social work services to patients, including assessments, care plan development, resource coordination, and ongoing support and education to patients and families.
  • Collaborate with the interdisciplinary team to ensure optimal patient outcomes.
  • Develop and implement policies and procedures to support the growth and success of the organization.
  • Train and onboard new team members, ensuring they have the necessary skills and knowledge to excel in their roles.

Requirements

  • Clinical social work skills, including assessment, treatment planning, and crisis intervention.
  • Minimum two years of experience in dialysis.
  • Strong leadership and management skills, with the ability to motivate and guide a team of LMSWs.
  • Excellent communication and problem-solving skills.
  • Knowledge of community resources and services.
  • Ability to work collaboratively with an interdisciplinary team.
  • Comfort using electronic health records and other technology platforms.
  • Master's degree in Social Work from an accredited institution and active Michigan licensure as a Licensed Master of Social Work (LMSW).
