Clinical Management Consultants is seeking a highly skilled MDS Nurse Coordinator to join our team. As a key member of our healthcare team, the MDS Nurse Coordinator will be responsible for coordinating patient care activities, implementing policies and procedures, and evaluating patient care activities.
Key Responsibilities:
- Coordinate and ensure the completion of interdisciplinary patient care plans for patients admitted to the Transitional Care Unit.
- Implement and maintain accurate and up-to-date patient records.
- Collaborate with healthcare professionals to ensure seamless patient care delivery.
- Develop and implement policies and procedures to improve patient care outcomes.
- Conduct regular evaluations of patient care activities and report findings to the Director.
Requirements:
- Current licensure as a Registered Nurse (RN) in the state of New Mexico.
- Minimum of 2 years of experience in a healthcare setting, preferably in a patient care coordination role.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Strong analytical and problem-solving skills.
