Social Worker-Case Management Specialist

Ascension St. Vincent is seeking a skilled Social Worker-Case Management Specialist to join our team. In this role, you will coordinate the overall interdisciplinary plan of care for patients, from admission to discharge. You will create plans for care across the continuum, integrating patient/family preferences and values. As a resource, you will assess, evaluate, recommend, and treat legal, neglect, substance abuse, crisis, psychosocial, and various adjustment needs. You will advocate for resources and removal of barriers, maintaining ongoing dialog with supervisors and care transition team members to ensure effective implementation and reevaluation of health plans. You will also act as a resource for adequate medical record documentation, complex level of care recommendations, and services as they relate to diagnoses and treatment options for post-discharge care.
Requirements
Licensure / Certification / Registration:
One or more of the following required: Clinical Counselor, Counselor, Social Worker credentialed from the Indiana Behavioral Health and Human Services Licensing Board, or Social Worker specializing in Clinical Practice credentialed from the Indiana Behavioral Health and Human Services Licensing Board. Case Manager credentialed from the Commission for Case Manager Certification or American Case Management Association is preferred.
Education:
Bachelor's degree in Social Work required.
Work Experience:
3 years of experience preferred.
