Job Title: Admissions RN

Join Interim HealthCare of the Twin Cities as an Admissions RN and take advantage of a rewarding career in home care. As a skilled nurse, you will be responsible for providing high-quality patient care in the comfort of their own homes.

About the Role:
  • Complete admissions for the Home Care team, ensuring a smooth transition for patients and their families.
  • Direct patient care under the supervision of the Director of Nursing or Nursing Supervisor, providing personalized attention and support.
  • Create and follow individualized plans of care, tailoring treatment to meet each patient's unique needs.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and optimal patient outcomes.
Requirements:
  • Licensed to practice as an RN in the state of Minnesota, with an active and good-standing license.
  • Able to travel within the geographic area serviced by the office, with a valid driver's license, reliable transportation, and proof of auto insurance.
  • Two years of experience as an RN, with a preference for hospice or homecare experience.
  • CPR Certification.
