We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Admission Nurse, working on a weekend schedule. As a key member of our home health care team, you will be responsible for conducting admission and start of care visits in the Twin Cities area.
Key Responsibilities- Conduct thorough assessments of clients to determine their home health care needs and develop individualized plans of care.
- Evaluate and assess clients for home health services, including the creation of their plan of care.
- Collaborate with clients, families, physicians, and other members of the home care team to ensure seamless care delivery.
- Recommend services from other disciplines that have not yet been ordered by the physician.
- Licensed as a Registered Nurse in the state of Minnesota.
- Driving is required; must have a reliable vehicle with current insurance and a valid Driver's License.
- Prior Home Health Nursing experience.
- Proficiency with Medicare & OASIS documentation.
- Demonstrated ability to work autonomously.
- Excellent at planning, organizing, and communicating.
