Job Summary

The Care Team Specialist Consultant plays a pivotal role in delivering exceptional patient experiences at La Maestra Community Health Centers. As a vital member of our team, you will be responsible for greeting patients, clients, and visitors with a warm and welcoming smile. Your duties will include checking out patients following their appointments, ensuring all self-pay and third-party insurance information is accurately processed, and providing personalized care plans to connect them with necessary services. Strong customer service skills are essential for this role, as you will be the face of our organization and interact with patients on a daily basis.

Responsibilities:

  • Greet patients, clients, and visitors upon check-out, adhering to our organizational customer service standards.
  • Efficiently process patient check-outs using NextGen enterprise practice management (EPM) auto flow processes, updating demographics and scheduling follow-up appointments as needed.
  • Print and distribute Patient Plans through our electronic health record (EHR) system.
  • Provide printed Patient Education materials to patients as required.
  • Distribute information regarding new medications to patients during check-out.
  • Create follow-up and preventive screening appointments for patients after their provider visit.
  • Generate Same/Next Day reports to prepare for upcoming patient appointments and identify additional services they may qualify for.
  • Answer telephones, schedule appointments, and provide clinic operation information while maintaining La Maestra's established Phone Call Etiquette.
  • Schedule appointments across departments and sites using the PCMH script, correcting scheduling errors as necessary.
  • Assign and reassign patients to Primary Care Providers as needed.
  • Reschedule appointments when providers are unavailable, following the PCMH script.
  • Maintain La Maestra's Phone Call Etiquette at all times.
  • Check voicemail messages and take appropriate action, such as calling back patients or scheduling appointments.
  • Notify patients of insurance coverage changes and arrange payment options according to our sliding fee scale.
  • Familiarize yourself with government programs and health insurances we participate in, assisting patients with understanding their coverage.
  • Scan and archive documents into patients' charts in a timely manner.
  • Contact patients if they failed to check out, scheduling preventive services as needed.
  • Input Imaging Referral orders into EHR for In-House ultrasounds and X-rays.
  • Generate referrals from the referral tab during patient checkout.
  • Schedule appointments for completed In-House Referral orders.
  • Verify that all Care Team information is correctly input by other staff members during patient checkout and distribute Patient Satisfaction Surveys.
  • Provide weekly productivity reports to your supervisor.
  • Attend daily huddles with your Care Team.
  • Perform other tasks as directed by your supervisor.
  • Maintain effective communication and working relationships with colleagues and patients.

Requirements:

Education: High School Diploma or GED

Certification: Medical Assistant Certificate preferred

Experience: 1-2 years of experience in a physician office or hospital setting as a Patient Services Representative or registration preferred

Verbal and Written Skills: Bilingual (English/Spanish) preferred; Familiarity with basic word processing, spreadsheet, and database applications; Accurate keyboarding skills
