Job Summary:
The Community Health Worker III (CHW-III) is a key member of our team at Providence California Regional Services, responsible for helping community residents and Medical Center patients and their families to navigate and access community services, other resources, and adopt healthy behaviors.
Primary Responsibilities:
- Health Education: Assist with recruitment and administrative tasks for health education courses, such as Mental Health First Aid, Creating Healthier Attitudes Today, physical activity, or Groceryships. Deliver these modules independently and identify new locations where the programs can be delivered.
- Outreach and Enrollment: Provide outreach in community settings and provide initial screening for eligibility of Covered CA, Medi-Cal, and CalFresh (Food Stamps). Assist with one-on-one health insurance and/or CalFresh enrollment application assistance. Troubleshoot complex cases independently and provide small group presentations on how to use insurance and basic health care rights.
- Navigation/Linkage to Resources: Manage complex patient subpopulations, including individuals with co-morbidities, recently discharged patients from the Medical Centers or community residents struggling with substance abuse. Provide individualized assessments of risk factors that impact an individual's well-being. Refer individuals to appropriate programs and resources in the community and provide follow-up to ensure needs are met.
Requirements:
- Education to meet certification, license or registration requirement
- Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request.
- 1 year experience in a combination Community Health Worker I and II position, or 2 years related community outreach, enrollment, education or other community health administrative experience, or Bachelors degree in lieu of experience
