The Patient Coordinator will be responsible for managing the clinical support coordination of the patient's care under direct supervision of the Practice Development Manager. Primary responsibilities include scheduling follow-up appointments, surgery coordination, communication across care teams, and other non-clinical activities in support of patient care.

This is a full-time position working directly with an Upper Extremity Physician and Team. Scheduled day hours are 7:30 AM to 4:00 PM (*hours are approximate/estimated based on team/clinic needs). Primary locations will be our Plymouth, Eden Prairie, Maple Grove, and Minnetonka campuses. Flexibility to float to other locations as needed.

This position may involve working at any Summit location and/or outreach sites, as assigned. The estimated salary for this role is approximately $55,000-$65,000 per year, depending on experience.
