As a Hospice RN Case Manager, you will:

  • Report directly to the Administrator, Executive Director, or Patient Care Manager Senior, maintaining primary oversight and professional management of each patient.
  • Serve as the main liaison among the physician, patient/family, and hospice team.
  • Deliver direct patient care services to hospice patients and provide supportive care to their families while managing a reasonable caseload and fostering a trusting and compassionate relationship.
  • Empower caregivers by educating them on how to care for their loved ones.
  • Collaborate with an interdisciplinary team, including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to ensure comfort, peace, and dignity.
  • Assess patient and family needs and assist in developing and executing individualized care plans.
  • Maintain all assigned patient clinical records.
  • Participate in quality assessment and performance improvement initiatives.


Candidate Profile:

We are looking for a Hospice RN who is compassionate, kind, and empathetic towards patients and their families.

Key qualifications include:

  • RN license in the state of employment (or eligibility to obtain).
  • Minimum of 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
  • Additional experience in hospital settings such as Med Surg, PACU, Float Pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing is advantageous.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
