Position Overview:

The Care Partner will deliver individualized support and services to residents, embodying the principles of assisted living, and enhancing the overall quality of life for the residents. Care Partners are responsible for assisting and guiding residents in their daily activities with professionalism and compassion. Responsibilities are to be executed in compliance with current Federal, State, and local regulations, as well as the standards established by Oaks Senior Living and the employee handbook.

Key Responsibilities:

Resident Care

1. Deliver exceptional care and support to residents in alignment with the Oaks Senior Living philosophy of Person-Centered Lifestyle and respect for resident rights.

2. Aid residents with daily living activities tailored to their individual preferences.

3. Safeguard the confidentiality of personal and medical information pertaining to residents and colleagues.

4. Follow each resident's Personalized Service Plan; inform the Shift Supervisor, Manager on Duty, Wellness Director, and/or Executive Director of any changes in the resident's health status.

5. Respond promptly to resident requests for assistance.

6. Facilitate resident engagement in activities, encouraging participation.

7. Support residents during meal times, including escorting them to and from the dining area, serving meals, and assisting with dining needs while observing and reporting any changes in appetite or physical capabilities.

8. Assist in maintaining cleanliness in the dining area post-meal service as required.

9. Help keep residents' living spaces tidy and organized.

10. Ensure cleanliness and hygiene in resident, employee, and public restrooms as necessary.

11. Build and maintain positive relationships with residents and their families.

12. Dispose of all personal care products in accordance with established policies.

13. Utilize proper body mechanics when assisting with resident transfers and adhere to safety protocols outlined by Oaks Senior Living.

14. Act in response to resident emergencies following established procedures.

15. Promote choice, dignity, and individuality for all residents.

16. Uphold and protect resident rights.

17. Complete 24 hours of continuing education in the first year of employment and 16 hours annually thereafter. Maintain certifications in CPR and First Aid.

18. Follow infection control policies and established safety measures.

19. Participate in special events, activities, and promotional programs.

20. Perform additional duties as assigned by the Shift Supervisor, Wellness Director, Manager on Duty, or Executive Director.

Communication:

1. Attend all scheduled staff meetings and mandatory training sessions.

2. Communicate effectively to relay information to residents, Shift Supervisor, and Management.

3. Review daily any communication tools utilized in resident care.

4. Convey residents' needs to the Shift Supervisor and caregiving staff during shift changes.

5. Alert your supervisor or the Wellness Director if you notice any changes in a resident's condition.

6. Report all supply requirements.

7. Notify the Executive Director of any hazardous conditions or equipment issues.

8. Address on-the-job injuries in accordance with community policies and procedures.

9. Inform your supervisor of any concerns or inquiries raised by residents' family members.

10. Maintain a positive and professional demeanor.

11. Carry out other caregiving responsibilities as assigned.

Reports to: Shift Supervisor, Wellness Director, Manager on Duty, and Executive Director.

Qualifications:

1. Previous experience in caregiving for older adults is preferred.

2. Ability to read, write, and communicate effectively in English.

3. A genuine desire to work with older adults, demonstrating a friendly and caring attitude.

4. Must be at least 18 years old and pass a criminal background check.

5. Must undergo a physical examination by a licensed physician and pass a drug screening.

6. Awareness of the risks associated with exposure to infectious waste, diseases, chemicals, and disinfectants.

7. Ability to respond effectively in emergency situations.

Physical Job Requirements:

To meet job requirements, employees must be capable of performing essential physical tasks for extended periods. Employees must adhere to proper body mechanics and safety protocols when assisting or transferring residents. In a typical eight-hour workday, employees will:

1. Stand or walk for up to eight hours.

2. Sit for up to two hours.

3. Frequently support up to 70 pounds.

4. Occasionally lift or carry up to 50 pounds.

5. Frequently kneel, bend, and reach.

6. Secure appropriate assistance for resident transfers as needed.
