Overview
As a Community Support Specialist, you will collaborate with the Social Services Director/Coordinator to identify and address each resident's social, emotional, and psychological needs, while fostering their overall development during their stay at our facility and assisting in discharge planning.
Your Impact
- Provide support to residents and their families regarding social, emotional, and familial concerns.
- Ensure timely and accurate completion of documentation in compliance with Facility, State, and Federal regulations.
- Engage in resident care planning and discharge planning processes.
- Act as a reliable source of information for residents and their families.
Qualifications
- Minimum of one (1) year of experience in social work, preferably within a healthcare environment.
- High level of professionalism and ability to maintain confidentiality.
- Strong verbal and written communication skills.
