About the Job

As a Resident Service Coordinator at NHE, Inc., you will play a vital role in supporting our residents to live independently and age in place. This is a part-time position with an estimated salary of $24.50 per hour.

Job Summary

We are seeking a highly skilled and compassionate individual to join our team as a Resident Service Coordinator. In this role, you will be responsible for educating and providing referrals and services to residents who require support. You will empower residents to make informed decisions about their care and connect them with community-based resources.

Key Responsibilities

  • Conduct comprehensive assessments of residents to identify their needs and develop personalized plans for support.
  • Connect residents with local services and providers that meet their needs, including personal care services.
  • Maintain relationships with service providers and ensure seamless communication between residents, caregivers, and healthcare professionals.
  • Monitor resident progress and adjust plans as needed to ensure optimal outcomes.
  • Participate in ongoing training and professional development to stay up-to-date on best practices and industry developments.

Requirements

To be successful in this role, you must possess a bachelor's degree in social work or a related field and have at least two years of experience in social services or a related field. Strong communication and interpersonal skills are essential, as well as the ability to work independently and manage multiple priorities. Knowledge of local services and resources is also highly desirable.
