Position Title: Team Manager

Division: Community Living/Services

Supervisor: Assistant Director of Community Living

Classification: Non-Exempt

Purpose:

Oversee direct care personnel and manage the daily operations and upkeep of the group home. Ensure effective communication within the Agency and the Community Living Services division. The primary goal is to create a living environment that is as typical as possible while promoting the development of each client towards maximum independence.

Key Responsibilities:

  1. Supervise direct care staff to guarantee the implementation of each client's Individualized Program Plan (IPP).
  2. Provide training to staff across all programs utilizing various instructional methods.
  3. Coordinate the provision of training and rehabilitation services.
  4. Monitor the execution and follow-up of each client's IPP.
  5. Schedule and lead interdisciplinary staff meetings for program planning and evaluation.
  6. Oversee documentation and record-keeping.
  7. Engage directly in recreational, therapeutic, and training activities with clients.
  8. Act as a liaison among clients, families, staff, professionals, and the community.
  9. Protect the human and civil rights of clients and families.
  10. Manage medication supplies to ensure adequacy.
  11. Assist clients in managing their finances.
  12. Maintain sufficient household supplies.
  13. Accurately manage petty cash.
  14. Support clients with their programming needs both formally and informally.
  15. Deliver services in accordance with State Board of Health regulations.
  16. Encourage client participation in community events.
  17. Exhibit patience in all interactions with clients, staff, parents, and community members, using a calm and positive tone.
  18. Meet the physical demands associated with client care, which may include lifting, guiding, and assisting with physical transfers.
  19. Ensure compliance with all departmental and agency policies as well as state and federal regulations.
  20. Respond to client issues as they arise.
  21. Maintain a safe and secure environment.
  22. Utilize personal or company devices for time entry as necessary.
  23. Ensure all medical needs of clients are met, including transportation to appointments and communication with nursing staff.

Essential Skills:

  1. Implement programs promptly.
  2. Conduct staff meetings as required.
  3. Document and provide input for monthly provider/residential meetings.
  4. Route receipts to the Administrative Assistant.
  5. Replenish household supplies as necessary.
  6. Support direct care staff effectively.
  7. Provide direct care for a minimum of 20 hours weekly.
  8. Document personnel issues and communicate them to the QIDP or Director of Residential.
  9. Complete tasks assigned by supervisors.
  10. Accept guidance from supervisors and management.
  11. Maintain confidentiality regarding all clients and staff.
  12. Coordinate with the business office to ensure liability payments are processed.

Requirements:

  1. Bachelor's degree in a human services field preferred; High School Diploma or equivalent required.
  2. Minimum of one year of experience with the developmentally disabled population, ideally in a residential context.
  3. Ability to obtain and maintain certifications in CPR, First Aid, NCI, and Medication Administration.
  4. Excellent oral and written communication skills with a commitment to confidentiality.
  5. Possess a valid driver's license and maintain insurability as per agency standards.
