**Key Responsibilities:**

  • Develop and implement personalized home care plans that cater to patients' unique needs and goals.
  • Provide occupational therapy services to patients in their homes, promoting independence, mobility, and overall well-being.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and communication.

**Requirements:**

  • Bachelor's degree from an accredited school approved by the American Occupational Therapy Association.
  • Active OT license in the state where you practice.
  • Current CPR certification.
  • One year of occupational therapy experience; additional home health experience preferred.

**Salary Range:** $35.29 - $51.17 per hour
