Nurse Practitioner

Job Summary:
Oak Street Health is seeking a highly skilled Nurse Practitioner to join our team as an Advanced Practice Provider. As a key member of our primary care team, you will provide exceptional patient care, working collaboratively with physicians, nurse practitioners, and other healthcare professionals to deliver high-quality, patient-centered care.
Responsibilities:
- Provide routine and acute care to patients, including conducting office visits, administering annual wellness visits, and health risk assessments.
- Collaborate with other providers, specialists, testing facilities, and agencies to ensure comprehensive care.
- Lead population health initiatives, including coordinating care with the Care Team to ensure patients receive evidence-based care.
- Assist with phone triage and outreach, educating patients on their health conditions, care plans, and promotional activities.
- Conduct home visits as needed and perform other duties as assigned.
Requirements:
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
- National certification in at least one of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
- Active, non-probationary state Nurse Practitioner license and DEA license.
- US Work Authorization.
Preferred Qualifications:
- Experience in primary care: internal medicine, geriatrics, or family medicine.
- Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
- Passion for teamwork and collaboration.
- Bilingual proficiency in applicable areas.
