Job Summary

We are seeking a skilled Registered Nurse to join our team at Home Care by Black Stone. As a Registered Nurse, you will provide high-quality clinical services to patients in their homes, working closely with other healthcare professionals to achieve and maintain continuity of patient care.

Key Responsibilities
  • Provide clinical services within the scope of practice and in accordance with infection control standards, in coordination with other members of the patient's care team.
  • Complete clinical nursing assessments in accordance with federal and/or state program requirements.
  • Ensure patient eligibility and medical necessity for services as defined by payer source and agency policy.
  • Develop and revise individualized plans of care and/or service plans according to federal and/or state program requirements with other community providers.
Requirements
  • A minimum of one year experience as an RN is preferred.
  • Current RN licensure in the state of practice is required.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation are required.
  • Current CPR certification is required.
