Key Responsibilities:
  • Provide case management services to clients, including assessment, planning, and coordination of treatment services.
  • Conduct crisis intervention and provide support to clients in crisis.
  • Facilitate support groups and provide education on life skills and other relevant topics.
  • Advocate on behalf of clients to obtain needed services and resources.
  • Interfacing with agencies and systems to assist clients in meeting mandates and accessing transitional support services.
  • Conduct ongoing case management related to housing, transportation, childcare, food, and other related needs.
  • Maintain communication with direct service providers and vocational service providers.
  • Submit weekly schedules and monitor effective utilization of time.
  • Attend and participate in staff/community meetings as needed.
  • Provide backup coverage as needed.
  • Maintain computer skills and exercise due diligence in the delivery of quality care in line with agency Medicaid Compliance Plan and systems of accountability.
Requirements:
  • 38000-50000 Yearly Salary
