Job Title: Foundation Medical Partners

Job Summary:

We are seeking a highly skilled and compassionate Family Medicine Physician to join our team at Foundation Medical Partners. As a key member of our healthcare team, you will be responsible for providing high-quality, patient-centered care to individuals of all ages.

Responsibilities:

  • Conduct comprehensive physical examinations and provide preventative care services, including screenings and immunizations.
  • Order and interpret diagnostic tests, such as lab results and X-rays, to inform diagnoses.
  • Prescribe medications and manage medication regimens effectively.
  • Provide education and counseling to patients on healthy lifestyle choices and disease management.
  • Collaborate with specialists and other healthcare professionals to ensure seamless continuity of care.

Qualifications:

  • Board-certified or board-eligible in Family Medicine.
  • Current NH medical license or ability to obtain.
  • Minimum of 2 years of experience in a primary care setting.
  • Proficiency in EPIC EMR.
  • Excellent communication and interpersonal skills.
