Job Title: Foundation Medical Partners
Job Summary:
We are seeking a highly skilled and compassionate Family Medicine Physician to join our team at Foundation Medical Partners. As a key member of our healthcare team, you will be responsible for providing high-quality, patient-centered care to individuals of all ages.
Responsibilities:
- Conduct comprehensive physical examinations and provide preventative care services, including screenings and immunizations.
- Order and interpret diagnostic tests, such as lab results and X-rays, to inform diagnoses.
- Prescribe medications and manage medication regimens effectively.
- Provide education and counseling to patients on healthy lifestyle choices and disease management.
- Collaborate with specialists and other healthcare professionals to ensure seamless continuity of care.
Qualifications:
- Board-certified or board-eligible in Family Medicine.
- Current NH medical license or ability to obtain.
- Minimum of 2 years of experience in a primary care setting.
- Proficiency in EPIC EMR.
- Excellent communication and interpersonal skills.
