**Job Overview:**

We are seeking a highly skilled and dedicated professional to join our team as a Public Health Nurse, Registered Nurse or Public Health Social Worker. The successful candidate will be responsible for assessing the needs of individuals and communities, providing education and health promotion counseling, and coordinating with healthcare providers to deliver quality services.

Responsibilities:

  • Assess the health status and safety of individuals and families across the lifespan in various settings, including homes, clinics, and community centers.
  • Develop and implement care plans in coordination with physicians, service providers, clients, and family members.
  • Provide ongoing evaluation of individual and family health status, monitor progress towards goals, and respond to treatment and service needs.
  • Make referrals to appropriate providers, order equipment and supplies, and communicate with physicians regarding care plans.
  • Maintain complete and accurate documentation, adhering to industry regulations, standards, and contracts.
  • Collaborate with the Minnesota Department of Health and Regional Epidemiologist on surveillance, investigation, treatment, follow-up, and enforcement of communicable disease outbreaks.
  • Implement acute and chronic disease prevention and control measures, including immunization clinics, blood pressure and diabetic screening, and vaccinations.
  • Teach and collaborate with others in offering health education programs for individuals and groups, increasing knowledge and self-awareness about health and safety.
  • Participate in agency staff meetings, assist with education and training of nursing students, and maintain confidentiality and universal precautions.

Requirements:

  • Bachelor's degree in nursing, social work, human services, sociology, psychology, or equivalent combination of education and experience.
  • CPR certification, driver's license valid in the State of Minnesota, and relevant licensure (PHN, RN, or Social Work).
  • Ability to maintain certifications through ongoing education and possess strong communication and problem-solving skills.

Salary: $31.46-$44.48 per hour for RNs and $33.36-$47.14 per hour for PHNs/Social Workers, plus comprehensive benefits package.
