We are seeking a compassionate and skilled Home Care Registered Nurse to join our team at the Good Samaritan Society. As a Home Care RN, you will provide high-quality patient care and support to seniors in their homes, promoting independence and dignity.
Responsibilities- Provide comprehensive nursing care to patients in their homes, including assessments, medication management, and wound care.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
- Conduct regular home visits to monitor patient progress and provide education on self-care and disease management.
- Document patient information and care plans accurately and thoroughly.
- Current RN licensure in the state of practice.
- Minimum 3 months of experience in Long Term Care or Home Health nursing.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Strong organizational and time management skills.
