Job Title: Case Manager - Co-Responder Program

At Mental Health Resource Center, we are seeking a highly skilled Case Manager to join our Co-Responder Program team. As a Case Manager, you will work closely with individuals in need of services, assessing their needs and coordinating a plan of care and/or treatment plan.

Responsibilities:
  • Deliver case management services to individuals in need of ongoing support and services.
  • Conduct initial and ongoing assessments to identify individual needs and strengths.
  • Develop and implement care plans and/or treatment plans that address individual needs and utilize individual strengths.
  • Coordinate community referrals and services to support individuals in living in the community in the least restrictive setting.
  • Monitor the effectiveness of community services and provide outreach at the jail to assist inmates with developing a plan for care upon release.
Qualifications:
  • Bachelor's degree in Social Work or a related Human Services field from an accredited university or college.
  • One year of experience working in human services or a mental health related field.
  • Proficiency in the RBHS/MHRC Electronic Health Records (EHR) and Patient Information System.
  • Strong communication skills and ability to interact appropriately with internal and external customers, families, caregivers, community service providers, supervisory staff, and other department professionals.
