We are seeking a compassionate and strengths-based Family-Centered Care Coordinator to join our team at LSS of South Dakota.
About the Role:
- This position involves implementing social work case management to children, youth, and their families by facilitating Family Team Conferencing and the Wraparound process.
- The successful candidate will promote a family driven, youth guided, community based, culturally competent and trauma informed system of care.
The Family-Centered Care Coordinator is responsible for:
- Facilitating Family Team Conferencing and the Wraparound process to support youth and families facing challenges.
- Seeking appropriate community resources for families and monitoring the quality of services provided.
- Collecting and documenting required data to ensure the process is outcome-driven and model fidelity is upheld.
To be successful in this role, you should have:
- A Bachelor's Degree in Social Work or a related field.
- One to two years of related experience.
- Licensure as a Social Worker or Social Work Associate through the South Dakota Board of Social Work Examiners.
