Responsibilities

The Family-Centered Care Coordinator is responsible for:

  • Facilitating Family Team Conferencing and the Wraparound process to support youth and families facing challenges.
  • Seeking appropriate community resources for families and monitoring the quality of services provided.
  • Collecting and documenting required data to ensure the process is outcome-driven and model fidelity is upheld.
Qualifications

To be successful in this role, you should have:

  • A Bachelor's Degree in Social Work or a related field.
  • One to two years of related experience.
  • Licensure as a Social Worker or Social Work Associate through the South Dakota Board of Social Work Examiners.
