Position Overview:

The Nurse Practitioner - Advanced Illness Management (AIM) CPOM is responsible for facilitating advanced illness management and palliative care for patients, ensuring a comprehensive and tailored treatment plan while demonstrating exceptional clinical skills and ethical judgment. This role involves providing diagnosis, treatment, consultation, and follow-up under the guidance of the Professional Services Corporation Physician.

Key Responsibilities:

  • Collaborate with the Professional Services Corporation (PSC) Physician, Primary Care Physician, and other members of the palliative care team to deliver advanced illness management and palliative care through diagnosis, treatment, and follow-up for referred patients.
  • Oversee advanced illness management from initial assessment to discharge, including evaluating disease-specific changes, functional decline, nutritional status, pain management, and the overall impact of illness on the patient's and family's quality of life.
  • Develop individualized care plans in consultation with patients, families, and the Advanced Illness team, including the Primary Care Physician.
  • Facilitate access to necessary care as identified in the care plan.
  • Participate in on-call services as required, providing competent and responsive care.
  • Prescribe medications, including controlled substances, as permitted by state law, and order diagnostic tests and treatments as needed.
  • Communicate clinical findings to the PSC Physician regularly and reassess the effectiveness of care plans.
  • Maintain effective working relationships with the supportive care team and involve them as necessary to achieve desired patient outcomes.
  • Document clinical findings, treatment plans, and care provided in the patient's medical record in accordance with established standards.
  • Stay informed about Palliative Medicine Quality Measures and ensure proper documentation and reporting.

Qualifications:

Required Skills and Knowledge:

  • Strong leadership, interpersonal, and communication skills.
  • Ability to make independent decisions and demonstrate self-motivation.
  • Understanding of Palliative Medicine principles, Medicare/Medicaid regulations, and value-based care requirements.
  • Compliance with continuing education requirements.
  • Proficiency in electronic medical record systems.
Education and Experience:
  • Master's degree in Nursing from an accredited institution or equivalent.
  • At least two years of experience as a Nurse Practitioner.
  • Preferred specialty in Family or Adult Medicine.
  • Experience in Value-Based Care practices.
  • Familiarity with Part B billing.
  • Able to work independently in an Advanced Practice Role.
Licenses and Certifications:
  • Current RN license or equivalent as required by state law.
  • Current Nurse Practitioner certification/license.
  • Collaborative agreement with a supervising physician as required by state law.
  • Part B Provider certification.
  • Federal and state DEA licenses as required.
  • Preferred ACHPN certification in Hospice and Palliative Care.
  • Valid driver's license and automobile insurance.
