Key Responsibilities

In your role as a Pharmacist, you will be responsible for:

  • Assessing and overseeing drug therapy to meet the comprehensive needs of home care patients in collaboration with healthcare professionals.
  • Ensuring compliance with organizational policies and relevant state and federal pharmacy regulations.
  • Participating in quality assurance initiatives and adhering to accreditation standards.
  • Facilitating new patient referrals and admissions through effective communication with nursing staff and customer service.
  • Reviewing and verifying patient orders, documenting them accurately.
  • Collaborating with physicians and healthcare teams to:
    • Plan and implement home patient care.
    • Establish therapeutic goals for medication management.
    • Monitor the safety and effectiveness of drug therapies.
    • Prepare detailed progress reports.
  • Conducting thorough assessments of patient, medication, and disease information to identify potential drug-related issues.
  • Monitoring drug therapy outcomes by analyzing laboratory and clinical data.
  • Providing expert consultation to patients, families, and healthcare professionals regarding medication use.
  • Maintaining accurate documentation of all clinical interactions and activities.
  • Processing and verifying medications and supplies prior to dispensing.
  • Staying current with pharmacotherapy knowledge through ongoing education and professional development.

Qualifications

The ideal candidate will possess:

  • A Bachelor of Science in Pharmacy; a Doctor of Pharmacy is preferred.
  • A valid Registered Pharmacist License in the relevant state.
  • A minimum of two years of experience in an acute care environment or one year in home infusion therapy.
  • Experience in compounding infused medications.
  • A proven track record in evaluating and managing drug therapies for home care patients.
  • Strong organizational and time management skills.
  • Excellent verbal and written communication abilities.
  • A flexible schedule with availability for on-call duties as needed.
