Job Summary

We are seeking a highly skilled and compassionate Clinical Nurse Manager to join our team at Community of Hope. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and leading our nursing staff in delivering high-quality services.

Key Responsibilities
  • Develop and implement patient-centered care plans to ensure seamless coordination of care across healthcare providers and settings.
  • Supervise and mentor nursing staff to ensure excellent patient care and adherence to clinical protocols.
  • Collaborate with multidisciplinary teams to improve patient outcomes and enhance the overall patient experience.
  • Stay up-to-date with the latest clinical guidelines and best practices to ensure evidence-based care.
  • Contribute to the development and implementation of policies and procedures to promote a culture of excellence and safety.
Requirements
  • Bachelor of Science degree in Nursing required.
  • Current, unencumbered DC Registered Nurse license with current CPR certification required.
  • One year of supervisory experience and one year working in an ambulatory, primary care, or med/surg setting.
  • Care management and patient education experience preferred.
  • Ability to work with computers and electronic health records required.
