Job Summary

Mountain View Hospital is seeking a skilled Pharmacist to join our team. As a key member of our healthcare team, you will be responsible for interpreting physician prescriptions, acting as a drug information resource, and compounding and dispensing medications.

Responsibilities
  • Review and interpret physician prescriptions to ensure accuracy and determine formulas and ingredients needed.
  • Compound medications using standard formulas and processes, including weighing, measuring, and mixing ingredients.
  • Monitor patient medication orders to ensure accuracy, drug allergies, drug interactions, duplicate therapy, and incompatibilities.
  • Check patient medication orders prior to delivery to patient care areas to ensure the correct drug, dosage form, schedule, and strength is being dispensed.
  • Perform all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors.
  • Prepare IV admixtures and extemporaneous parenteral products under aseptic conditions.
  • Direct Pharmacy Department personnel engaged in admixing, packaging, and labeling pharmaceuticals.
  • Answer questions and provide information to physicians, nurses, patients, and other healthcare professionals on the appropriate use of medications, drug interactions, side effects, dosage, storage, etc.
  • Assist in teaching programs (departmental, medical, nursing, and community).
  • Endorse policies and procedures of the department and of the hospital.
  • Ensure the maintenance and proper operation of equipment and supplies in the work areas.
  • Maintain established procedures concerning quality control, security of controlled substances, and disposal of hazardous waste.
  • Enter data into the computer system (Pharmacy program) such as patient, prescribed medications, and cost, and maintain patient profiles and charge system.
  • Instruct students and other medical personnel on matters pertaining to the Pharmacy Department.
  • Review and monitor drug therapy that requires dosing adjustments as authorized by the physician.
  • Provide pharmaceutical care to all ages of patients from newborns to the elderly.
  • Treat patients and their families with respect and dignity, identifying and addressing psychosocial, cultural, ethnic, and religious/spiritual needs.
  • Assist the Pharmacy Department Director on special assignments.
  • Supervise at least one to two members of the Pharmacy Department staff on a daily basis.
  • Assist the Pharmacy Department Director with responsibilities in accordance with the organization's mission, policy, and any applicable laws.
  • Interview and assist in the hiring process, train personnel, plan, assign, and direct work, address complaints, and resolve problems.
  • Demonstrate ability to read, analyze, and interpret pharmacy-related journals.
  • Demonstrate ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the hospital staff.
  • Demonstrate ability to write articles for publication and effectively present information to management, medical staff, hospital staff, and public groups.
  • Demonstrate ability to apply necessary advanced mathematical concepts such as exponents and logarithms.
  • Demonstrate ability to apply mathematical operations to such tasks as dosage calculations, formulation of pharmaceutical preparations, and data analysis.
  • Demonstrate ability to define problems, collect data, establish facts, and draw valid conclusions.
  • Demonstrate ability to interpret an extensive variety of technical instructions and deal with several abstract variables.
  • Complete annual education requirements.
  • Maintain regulatory requirements.
  • Maintain patient confidentiality at all times.
  • Report to work on time and as scheduled, and complete work within designated time.
  • Complete in-services and return in a timely fashion.
  • Attend annual review and department in-services, as scheduled.
  • Attend staff meetings in the absence of the Pharmacy Director and read and return all monthly staff meeting minutes.
  • Represent the organization in a positive and professional manner.
  • Actively participate in performance improvement and continuous quality improvement (CQI) activities.
  • Comply with all organizational policies regarding ethical business practices.
