Job Summary

We are seeking a skilled and compassionate Nurse to join our team as a Home Health Specialist. As a Home Health Specialist, you will be responsible for providing direct patient care, administering medications and treatments, and educating patients and their families on disease management and prevention strategies.

Key Responsibilities
  • Treatment and Care: Provide treatment and care to infants, children, and adolescents according to the active plan of care, as given by the physician and other appropriate personnel.
  • Medication Administration: Administer prescribed medication, treatments, and therapies as ordered by the physician.
  • Documentation: Document all activities, assessments, nursing actions, responses, and coordination of care.
  • Patient Education: Educate patients and their families regarding the disease process, self-care techniques, and prevention strategies.
Requirements
  • Nursing License: Active nursing license in good standing.
  • Experience: Six (6) months of recent experience as a nurse in a clinical care setting, with home health experience preferred.
  • Skills: Attention to detail, time management skills, effective problem-solving and conflict resolution, and good organization and communication skills.
