Pinnacle Home Care Inc. is seeking a dedicated and compassionate Physical Therapist to join our team of skilled home health care providers. As a Physical Therapist, you will play a vital role in providing quality care to our patients in their homes.
Job Summary:The Physical Therapist is a professional member of the healthcare team who provides Physical Therapy services to patients in compliance with all applicable practice acts. The Physical Therapist performs duties within standards of clinical practice as defined by the Department of Professional Regulation, the American Physical Therapy Association, and the policies and procedures of the agency.
Responsibilities:- Provides initial and ongoing comprehensive assessments of the client's needs, including Outcome and Assessment Information Set (OASIS) assessments.
- Establishes a treatment plan in consultation with the physician and revises it with the approval of the physician as necessary.
- Assists in the development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy.
- Teaches, supervises, and counsels the family and client in the physical therapy program.
- Graduate of an accredited Physical Therapy program or equivalent education.
- Licensed to practice as a PT in the State of Florida.
- Current BLS Certification.
- Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
- At least one year of experience as a Physical Therapist.
