Key Responsibilities:
  • Provide 24/7 hospice care to terminally ill patients and their families
  • Oversee and supervise all hospice care services, ensuring compliance with licensing regulations, certification standards, and legal and ethical standards
  • Develop, review, and revise patient/family plans of care, implementing and monitoring compliance with policies and procedures
  • Supervise the planning, coordination, and delivery of interdisciplinary group interventions
  • Assign patients/families to appropriate staff members, ensuring effective communication and collaboration
  • Provide leadership and counsel to nursing personnel, Hospice Aides, and Homemakers, enhancing staff development and performance
  • Participate in developing objectives for hospice, planning for employee orientation, and coordinating services with community agencies
  • Ensure compliance with federal, state, and local rules, accreditation standards, and hospice policies in all patient care aspects
Requirements:
  • Two years of hospice or home health experience preferred
  • Strong leadership and communication skills
  • Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities
  • Excellent problem-solving and critical thinking skills
  • Strong attention to detail and organizational skills
