Job Summary

La Clinica de Familia Inc. is seeking a highly skilled Behavioral Health Therapist - Integrated Care to join our team. As a key member of our Primary Care team, you will play a vital role in providing high-quality behavioral health services to our patients.

Key Responsibilities
  • Provide consultation services to primary care providers and patients to improve recognition, treatment, and management of psychosocial/behavioral problems and conditions.
  • Deliver brief, consultation-based services in the primary care clinic to patients and primary care providers using an integrated care model.
  • Collaborate with the primary care team to develop and implement effective treatment plans.
  • Conduct individual, group, and familial therapy sessions as needed.
  • Develop and maintain strong relationships with patients, primary care providers, and other healthcare professionals.
Requirements
  • Master's Degree in Social Work or a related field from an accredited college or university.
  • One of the following New Mexico licenses: LMSW, LMHC, LCSW, LPCC, LMFT.
  • Bilingual in English/Spanish.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment and prioritize tasks efficiently.
