Job Summary:

The Clinical Pharmacist will be responsible for providing clinical pharmacy services to patients in an ambulatory care setting. This includes collaborating with healthcare providers to monitor patient medication therapy, identifying potential medication-related problems, and developing and implementing plans to optimize patient outcomes.

Key Responsibilities:
  • Collaborate with healthcare providers to monitor patient medication therapy and identify potential medication-related problems
  • Develop and implement plans to optimize patient outcomes, including medication therapy management and disease state management
  • Provide education and counseling to patients on medication use and disease management
  • Participate in quality improvement initiatives to improve patient care and outcomes
  • Stay up-to-date with the latest clinical guidelines and research in pharmacy practice
Requirements:
  • Doctor of Pharmacy (Pharm.D.) degree from an accredited institution
  • Current, full, active, and unrestricted license to practice as a pharmacist
  • At least one year of full-time experience in clinical pharmacy practice, preferably in an ambulatory care setting
  • Board certification as a Pharmacotherapy Specialist or completion of an ASHP-accredited Ambulatory Care Residency
