Job Summary

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team as a Clinical Manager Assistant. This role will play a critical part in ensuring the continuity of high-quality care to home health patients assigned to the team's area.

Key Responsibilities
  • Assist the Patient Care Manager in coordinating a team of staff to provide quality care in an efficient manner.
  • Ensure that the team is committed to providing care in accordance with physician's orders and State/Medicare guidelines.
  • Prepare patient lists and necessary documents for weekly case conferences.
  • Provide direct patient care, as necessary, in accordance with scope of practice, plan of care, and physician orders.
  • Assist the Patient Care Manager in clinician documentation review/audits to ensure complete, accurate, and timely submission of all documentation by field staff.
Requirements
  • Current LPN licensure in the state of practice.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
