Job Overview

MGA Homecare, a leading provider of home and community-based healthcare services, is seeking an experienced Registered Nurse Case Manager to join our team.

Salary Information

The salary range for this position is $70,000-$75,000 annually. Additionally, you will be eligible for a monthly auto and cell phone allowance totaling $4,200.00 annually.

Key Responsibilities
  • Lead the care coordination process for clinical staff, patients, and their families/caregivers, as well as physicians or other care providers.
  • Develop and implement individualized plans of care for each client, ensuring timely and accurate documentation.
  • Provide education and support to patients and their families/caregivers to promote progress towards their goals.
Required Qualifications
  • A current state license as a Registered Nurse, BSN preferred.
  • Minimum one year of nursing experience required (two years preferred).
  • Demonstrate leadership capabilities with strong supervisory and interpersonal skills.
