Qualifications: High school diploma or equivalent is required.
Previous experience in a healthcare setting is preferred.
Familiarity with the practices and regulations governing operations in a post-acute care environment is essential.

Key Responsibilities:
- Implement and interpret departmental programs, objectives, policies, and procedures.
- Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation.
- Uphold the confidentiality of all proprietary and sensitive information.
- Adhere to company policies, including those related to harassment and compliance.
- Exhibit integrity and professionalism by following the organization's Code of Conduct and completing required compliance training.

Patient Support:
- Assist in the social services program to address the medically related social and emotional needs of patients, in alignment with Federal, State, corporate, and division guidelines.
- Ensure timely and appropriate documentation in accordance with State law.
- Collaborate with social worker(s) to support patients and families through education, financial planning assistance, and coordination with community agencies.

Communication Skills:
- Proficient in reading, writing, speaking, and understanding the English language.
