Responsibilities
  • Provide one-on-one care to pediatric patients in their homes, ensuring their medical needs are met and their well-being is prioritized.
  • Develop and implement individualized care plans, working collaboratively with patients, families, and healthcare teams to achieve optimal outcomes.
  • Administer medications, perform medical procedures, and provide wound care as needed, adhering to established protocols and guidelines.
  • Conduct thorough assessments, documenting patient progress and identifying areas for improvement.
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and coordinate services.
Requirements
  • RN/LVN with a current, valid license to practice as issued by the State Board of Nursing.
  • Current CPR certification.
