Job Title: Speech Language Pathologist

We are seeking a skilled Speech Language Pathologist to join our team at Forest Health & Rehab Center. As a Speech Language Pathologist, you will play a vital role in helping our patients achieve optimal physical function and facilitating their achievement of patient, physician, and therapy goals.

Responsibilities:
  • Screen and evaluate residents to determine their need for Speech, Language, or Swallowing Therapy.
  • Establish individualized treatment programs with consultation to facility staff in areas of Speech Therapy.
  • Provide results of evaluations and treatment objectives to caregivers, family members, and residents.
  • Complete all documentation as required by the fiscal intermediary, within the designated time frame.
  • Implement treatment programs based on evaluative results.
  • Monitor residents' responses to treatment and alter treatment plans as appropriate.
Requirements:
  • Master's Degree in Speech-Language Pathology from an accredited college or university and licensure accordingly.
  • Previous experience in long-term care is preferred but not required.
