Job Summary

Project Transition is seeking a dedicated and compassionate Residential Advisor to join our team. As a key member of our community, you will play a vital role in supporting individuals with serious mental illness, co-occurring substance use disorder, and/or dual diagnoses of SMI and IDD to live a meaningful life in the community.

Key Responsibilities
  • Develop and implement individualized training and treatment plans for assigned members, ensuring their unique needs are met and progress is documented.
  • Participate in crisis management systems, program activities, and supervision, providing a safe and supportive environment for our members.
  • Provide training and supervision in activities of daily living, promoting independence and self-sufficiency.
  • Transport members to and from appointments and recovery centers, ensuring timely and safe transportation.
  • Conduct and document face-to-face observations, apartment checks, and medication administration, maintaining accurate records and ensuring compliance with policies and procedures.
  • Prepare shift notes and monitor self-administration of medications, ensuring the well-being of our members.
  • Supervise Residential Advisors, providing guidance and support to ensure the highest quality of care.
  • Complete all required trainings and continuing education, staying up-to-date with best practices and industry developments.

Qualifications
  • High School Diploma or equivalent
  • Two years' experience in therapeutic community and/or psychiatric care
  • Valid Driver's License
