Job Summary

We are seeking a highly skilled Quality Care Coordinator to join our team at the Center for Primary Care. The successful candidate will be responsible for coordinating all aspects of patient care, working closely with patients, families, insurance representatives, and healthcare professionals to ensure high-quality care.

Key Responsibilities:

  • Support quality improvement initiatives, including data management, reporting, and health insurance incentive programs.
  • Monitor key clinical quality metrics, performance-based compensation reports, and other performance reports.
  • Collaborate with practices to identify areas for improvement and develop targeted plans to address these opportunities.
  • Identify providers for targeted outreach and quality initiatives.
  • Provide education and training to staff and providers on quality and population health initiatives.
  • Oversee clinical data collection, complete chart reviews, and upload records for Medicare, Medicare Advantage, and other insurance programs.
  • Support the practice in meeting organizational quality and efficiency goals through optimizing provider and patient engagement.

Requirements:

  • High School Diploma or equivalent required; Certified Medical Assistant (CMA) or Licensed Professional Nurse (LPN) licensure preferred.
  • Minimum two (2) years of recent clinical experience.
  • Strong working knowledge of clinical needs and disease processes for chronically ill populations.
  • Excellent communication, organizational, and analytical skills.
  • Ability to work autonomously and be directly accountable for assignments.

Work Environment:

This position is primarily based in a well-lighted office environment, with occasional local travel required.
