Job SummaryThe Staff Nurse will work in collaboration with our multidisciplinary team to provide comprehensive care to patients in our ambulatory care setting. This includes assessing patient needs, developing and implementing individualized care plans, and providing education and support to patients and their families.
Key Responsibilities- Assist in the care of patients in collaboration with the care team, providers, and clinical support staff.
- Perform triage in person or by telephone, and assist with translation and determination of acute issues.
- Document interventions and plans in the electronic medical record.
- Implement policies and procedures related to clinical conditions, and identify processes for improvement.
- Oversee and assist in the implementation of laboratory services, including point-of-care and HIV testing.
- Act as liaison for NJIIS/VFC and ensure that MA staff are entering and reconciling vaccines appropriately.
- Provide education and counseling on primary care services, including vaccine information and procedure review.
- Assess and assist in coordinating a response to community needs, such as outbreaks of illness or need for immunization.
- Attend health fairs and other outreach events.
- Provide patient education and document in the EMR.
- Conduct inventories and replenishments of medical and clinical supplies, and maintain high levels of confidentiality.
- Facilitate patient appointments and coordinate care among different providers and organizations.
- Successful completion of an accredited nursing program and a passing grade in the National Council Licensure Examination for Registered Nurses.
- Current and valid RN license from the state of New Jersey.
- Current BLS certification.
- Minimum of one year of experience in an ambulatory care/community health center setting.
- Experience working with community services agencies.
- Knowledge of Microsoft Office, keyboarding, and experience in the use of electronic medical records.
