Job Title: Registered Nurse Services Manager

VNA Health Group is seeking a highly skilled Registered Nurse to fill the role of Services Manager in our clinic. As a key member of our team, you will be responsible for providing exceptional patient care and ensuring the smooth operation of our clinical services.

Job Summary:

The Registered Nurse Services Manager will be responsible for coordinating clinical care for patients, collaborating with the healthcare team to provide appropriate referrals, and overseeing case management for medically complex patients. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Responsibilities:
  • Provide initial nursing assessment of new patients' needs and verify HIV status when necessary.
  • Coordinate clinical care for patients, including lab work, vaccines, and referrals to outpatient specialty care.
  • Oversee case management for medically complex patients, advocating for clients with insurers, subspecialty clinicians, and other healthcare providers.
  • Educate patients and families about pertinent medical issues, including assessment for referrals for PEP and PREP services.
  • Maintain familiarity with community resources to meet client needs.
  • Assist the team in identifying patient care improvement needs and participate in CQI activities.
  • Adhere to infection control and safety policies and guidelines of CHC regulatory agencies.
  • Provide timely and complete documentation in all relevant databases.
Qualifications:
  • Licensure as a Registered Nurse in NJ and a Nursing Degree from an NLN-accredited program.
  • Excellent interpersonal skills and computer skills.
  • HIV/AIDS work-related experience and knowledge of the LGBTQ community cultural and lifestyle issues are preferred.
  • Spanish bilingual or proficiency is preferred.
  • Current professional CPR/BLS certification is required.
Working Conditions:

This role requires working indoors and outdoors with equipment, including a thermometer, sphygmomanometer, stethoscope, otoscope, ophthalmoscope, EKG machine, venipuncture equipment, spirometry treatment, culture mediums, telephone, fax, photocopier, computer, printer, and automobile.

Sitting, standing, walking, bending, and lifting are required for this role.
