Job Summary
The RN Case Manager is a key member of the healthcare team at Metrowest Medical Center, responsible for facilitating care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources. This role requires a strong understanding of medical necessity and the ability to assess patients for transition needs to promote timely throughput, safe discharge, and prevent avoidable readmissions.
Key Responsibilities
- Integrate national standards for case management scope of services, including utilization management, transition management, and care coordination.
- Ensure that care is provided at the appropriate level of care based on medical necessity.
- Assess patients for transition needs and develop plans to promote timely throughput, safe discharge, and prevent avoidable readmissions.
- Collaborate with healthcare teams to ensure seamless transitions of care.
- Comply with state and federal regulatory requirements, as well as accreditation standards and company policies.
Requirements
- Graduate of an accredited School of Nursing.
- Current RN licensure in the State of Massachusetts.
- 2 years of RN acute care hospital experience.
- Acute care case management experience preferred.
- Accredited Case Manager (ACM) certification preferred.
